NCADA Hall of Fame Nomination Form

NCADA HALL OF FAME NOMINATIONS

Individuals may be nominated by any active or retired member of the NCADA. Self-nominations are not accepted for this award. Posthumous inductees will be represented by a family member.

Nomination Form – The official nomination form is available from the NCADA at www.NCADA.net. The form must be submitted online with appropriate support materials by October 15th of the current year for a candidate to be considered for the following year’s class.

Requirements for Consideration – In addition to the Official Nomination Form, the following materials should be submitted for an individual to be considered for induction:

A. Nomination Form: A completed digital nomination form.

B. Biography: Briefly describe the nominee’s achievements that meet the criteria for membership in the NCADA Hall of Fame.

  • Dates/Years of service in athletic administration at the local, state and/or national level
  • Honors, awards, special recognition for excellence in athletics
  • Additional information to support the nomination

C. All actions necessary to the selection process will remain confidential after the nomination form is received by the NCADA Hall of Fame Chairman.

D. Inductees must be present (or represented, if deceased) at the Hall of Fame awards ceremony. If unable to attend, the inductee will be placed into the next year’s induction ceremony.

Induction Ceremony

Section 1 The Hall of Fame Induction Ceremony is held each year at the NCADA Conference Banquet. Inductees will be notified by December 1st of their selection.

Section 2 When an individual who is selected for induction into the Hall of Fame cannot attend the current Induction Ceremony (unless deceased), that person will be offered the option of attending the following year’s Induction Ceremony.

Section 3 The NCADA will provide a Room on the night of the induction and banquet tickets for each inductee as well as up to ten family members.

Hall of Fame Award

Section 1         Hall of Fame Ring – To be designed by the EB and presented to the inductee provided by Southern Recognition Inc. Previous inductees have the option to purchase a ring at a reduced price.

Section 2         Plaque -To be distributed at the Awards Luncheon

Section 3         Lapel Pin – Provided by the Hall of Fame Committee
